The Healthy Dane Collaborative invites health care provider feedback in a new Community Health Needs Assessment survey. We want to gain insight on what you are seeing in your clinic patient population. We truly value your work and feedback. You can take the survey here.
As part of our work to improve community health outcomes, we are conducting a community health needs assessment. A Community health needs assessments (CHNA) creates an important opportunity to improve the health of our most vulnerable communities in Dane County. The CHNA is developed by the Healthy Dane Collaborative. The Collaborative includes UW Health, UPH-Meriter, SSM Health, Group Health Cooperative, Stoughton Hospital, and Public Health Madison Dane County.
Given the overlap amongst systems, you may receive this from other representatives. Only complete the survey one time. Your feedback will be kept anonymous. Please contact Adrian Jones, Program Manager Community Health Improvement in Office of Population Health, with any questions.
